What Everyone Wants to Know About Clitoral Hood Reductions – and Why It’s Often Recommended Alongside Labiaplasty

By Dr. Courtenay Poucher, MD – Labiaplasty Center of Los Angeles

If you’re researching labiaplasty or exploring options for improving the aesthetic and functional appearance of your intimate anatomy, chances are you’ve come across the term “clitoral hood reduction.” It’s a hot topic in forums on Reddit, in Facebook moms groups and on “girls night.”

Like any topic that is gaining in internet popularity, there are a lot of misconceptions and confusion about clitoral hood reduction.

Many women have questions about what the procedure actually involves, why surgeons recommend it and whether it affects sensation.

Today, I’m answering those questions clearly, compassionately and from the perspective of a board-certified specialist who has performed over 2,000 clitoral hood reduction procedures.

“What is a Clitoral Hood Reduction?”

The clitoral hood – also called the prepuce – is the natural fold of skin that covers and protects the clitoris. For some women, their hood is puffy or bulky; excessively large; has extra folds; asymmetrical; disproportionate to the labia minora; or covering the clitoris in a way that reduces pleasurable sexual stimulation.

It almost always connects directly to the back side of the labia minora, which is why more than 90 percent of labiaplasty cases also require a clitoral hood reduction.

In my practice at Labiaplasty Center of Los Angeles, a clitoral hood reduction removes small, precise amounts of that excess tissue to create a balanced, refined appearance while preserving the clitoris and all nerve structures.

To be clear: the clitoris itself is not surgically revised during a clitoral hood reduction, except in the case of a clitoropexy.

This is a highly delicate and technically skilled procedure, which is why choosing an experienced surgeon is essential.

“Why is Clitoral Hood Reduction Often Recommended with Labiaplasty?”

Not every patient needs a clitoral hood reduction, but it is extremely common to recommend it for the best cosmetic and functional outcome. Here’s why:

  1. It creates better symmetry

If the labia minora are reduced but the clitoral hood remains full or heavy, the final appearance can look incomplete or unbalanced. When both are treated together, the result is harmonious and proportional.

  2. It enhances the overall aesthetic result

Think of the labia and clitoral hood as one continuous structure. When one area is reshaped and the other is not, the contrast can be noticeable. Addressing both at once leads to a smooth, natural-looking contour.

  3. It can improve sexual sensation

Some women have excess clitoral hood tissue that restricts stimulation or creates a “buried” effect. Reducing some of that tissue – without exposing the clitoris too much – can make stimulation more accessible and comfortable.

  4. It prevents wrinkling or bunching after labiaplasty

After the labia are reduced, untreated excess hood tissue can appear more prominent. Removing that small extra fold avoids creasing or puffiness.

  5. It allows for a single recovery

Combining procedures minimizes cost and downtime. Most women prefer one healing period.

“Is the Clitoris Altered During a Clitoral Hood Reduction?”

It bears repeating, so I’m going to say it again: No, the clitoris is not surgically altered during a clitoral hood reduction.

A properly-trained surgeon never operates on, cuts or manipulates the clitoris itself in a clitoral hood reduction procedure. The procedure involves only the external skin folds.

Nerve pathways to the clitoris run deeper than the superficial tissue removed in a hood reduction. When performed correctly, sensation is preserved – and, for many women, improved.

“Does Clitoral Hood Reduction Affect Sexual Sensation?”

Yes, a clitoral hood reduction can positively affect sexual sensation, when performed correctly. Patients often report improved clitoral access, responsiveness, ease of stimulation and improved orgasm.

This is because removing excess skin reduces the “barrier” to the clitoris. Women who previously felt hidden, numb or restricted often experience a significant difference.

Temporary hypersensitivity during healing is normal and typically resolves within a few weeks.

“What does the Procedure Involve?”

At Labiaplasty Center of Los Angeles, clitoral hood reduction is a minimally-invasive, outpatient procedure that is painlessly performed under local anesthesia in our comfortable office. This portion of your day’s procedures is usually completed in about 30 to 45 minutes and dissolvable sutures are used.

“Is There a Visible Scar with Clitoral Hood Reduction?”

I can only speak for clitoral hood reductions performed in my office. In this case, my patients’ clitoral hood reduction scars are hidden naturally within existing folds of skin. Once healed, the results look incredibly natural and scars are usually not visible.

“Am I a Good Candidate for Clitoral Hood Reduction?”

You may be a great candidate if you are unhappy with excess or asymmetrical tissue over your clitoris; experience discomfort or irritation during intercourse and/or daily activities; feel your clitoris is too difficult to access during intimacy; and/or want the best cosmetic result from your labiaplasty procedure.

During your complimentary consultation, I will evaluate the labia and hood together to determine whether a combined approach would create the most balanced, aesthetic outcome.

“Why do Women Travel to Los Angeles for this Procedure?”

Each month, we treat women who fly in from Las Vegas, Canada, San Francisco, Texas, Sacramento, Europe, New York City and so many other locations because clitoral hood reduction requires exceptional precision and experience. When performed by a specialized surgeon, the results can be transformative – not just physically, but emotionally.
